Our nurses work with you and your doctor to provide the specialized care you need.
Skilled nursing care includes monitoring vital signs, administering medications, wound care, injections and infusion therapy. Our nurses are available 24/7, 365 days a year.
Patient and family teaching for a new or existing condition such as diabetes or heart disease is an important part of your nursing care. This includes recognizing important signs and symptoms, understanding when and how to take your medications and learning when to contact your doctor. We may utilize an in-home telehealth to provide your nurses and doctor with important daily information about your condition. This helps you remain healthy at home and reduce visits to the doctor or hospital.
Home Health Aides are available as needed for assistance with bathing, personal care and light household chores.
If needed, our Medical Social Worker provides support for social, emotional and financial aspects of your care and assists with access to community resources.
And, if you need an extra pair of hands around the house, our trained caregivers from Personal Care services are available.
Skilled nursing services provided under the direction of your doctor may be paid for by Medicare or other health insurance. Please find more information under Payment for Services.
