Hospice Care is a special program designed to assist people
of all ages who are facing a life threatening illness. We believe that hospice care is not about dying, but rather about living. Thus our focus of care, individualized to each patient and family member, incorporates pain relief, symptom management, emotional and spiritual comfort, and the resolution of end of life issues. With
compassion and dedication, we do our best to ensure that each
of our patients and their families are able to live each day
as fully as possible.
Our Hospice Program is Medicare Certified and Licensed by the California State Department of Health.
We provide:
- An interdisciplinary team of specially trained professionals to help care for the patient including a medical director, physician, nurse, and social worker as well as a chaplain, health aide, homemaker, and volunteers as needed.
- Services to people residing in skilled nursing and assisted living facilities.
- 24-hour, seven day a week on-call availability of a hospice registered nurse.
- Medications to provide comfort and symptom management related to the terminal illness.
- Instruction to aide loved ones in caring for the patient.
- Home Health Aide assistance with bathing and personal care.
- Support and practical assistance from trained hospice volunteers.
- Medical Equipment and supply.
- Emotional and spiritual support for patients and their loved ones.
- Counseling and bereavement support for surviving loved ones.
