Nursing interventions facilitate recovery after surgery, assist with managing chronic illness, medication monitoring, prevent rehospitalization and coordinate additional resources. We work together with you and your physician to ensure that you achieve the goals and desired outcomes of your plan of care.
Our nurses make “house calls” and can provide:
- Case Management
- Physical Assessments
- Education
- Infusion Therapy
- Wound Care
Social Services can assist with determining needed resources, access to community help, counseling support and financial assistance.
A Dietician is available to assist with special diets and to ensure that nutritional needs are met to achieve maximum recovery and health.
